It depends. In New York, Form IT-1099-R (the Summary of Federal Form 1099-R Statements) is only required to list distributions that had New York State, New York City, or Yonkers tax withheld. If only 2 of your forms had those taxes withheld, those are the ones that should be listed. The other 2 would not provide tax payment information so NY isn't interested in extra papers.
The federal government and NY already share income information.
