SEPP Calculation and Documentation

I am planning on taking SEPP distributions from my 401k plan. The plan administer does not assist with the calculation of the distribution amount. They merely ask for me to complete the form with the amount to be distributed on a monthly basis. So, I am assuming that I need to maintain a record or document of how I calculated the amount of my distribution. Is there any IRS guidance for requirements or best practice for calculating and documenting this calculation?
