Which code do I use for Box 12 on form W-2 if there are 4 different codes in box 12? My 4 codes are C,D,W,DD. Which one do I use if asked for only one?

You will be able to enter all of them, one on a line with the code at a time.  Then, you go down to the next line and select the code and enter the amount for the next one, etc. until all have been entered.
