Form 8606 : roth IRA conversion entries are incomplete for lines 6, 9 and 10.

My concern is the turbotax generated "Form 8606" do not have my entries on line 6. Please help me with how to fix or enter the details right.

I have forms  Form 1099-R (Back door Roth IRA conversion amount $6000) and Form 5498 ( for all Traditional IRAs and Rollover IRAs as IRAs year end 12/31/2019 balance - about $144,143). 

 

1. I filled in the 1099-R distribution on "Wages and income" (IRA, 401(k), Pension Plan Withdrawals (1099-R) )
2. After 1099-R filled, clicked "Continue" to follow all steps and enter the "Tell Us the Value of Your Traditional IRA" details. " all of my traditional IRA, SEP, and SIMPLE IRA accounts on December 31, 2019" - ($144,143.)

Despite I enter all info, the FORM 8606 entries for line 6 is missing. Please help how to update it right.

Form 8606 : roth IRA conversion entries are incomplete for lines 6, 9 and 10.

Notice the * (asterisk) next to line 15c.   That means that the line 6-12 calculations were done on the "Taxable IRA Distribution Worksheet" instead of the 8606 form per the instructions in the IRS Pub 590B (Worksheet 1-1) https://www.irs.gov/publications/p590b#en_US_2019_publink1000270091.

 

You can print all forms and worksheets to see that worksheet in TurboTax online.

 

(TurboTax sometimes uses that worksheet even when they do not need to, but that does not change the final calculations and taxable amount.)
