No, you do not report Form 5498 on your tax return - you won't find this form in TurboTax, nor do you file it with your tax return.

The institution that manages your IRA must report all contributions you make to the account during the tax year on the form. Depending on the type of IRA you have, you may need Form 5498 to report IRA contribution deductions on your tax return.
I have a 401k CD with a bank.
Due to COVID, no RMD for 2020 was issued.
So I did not have to declare RMD Income in 2021 tax return.
On 12/31/21, RMD was paid out to me.
The bank has issued ONLY a 5498, NO 1099-R. (The 5498 is dated 2020)
How do I report the RMD as Income on my 2021 Tax Return??
Please contact the bank to confirm that the payment was made in 2021. If it was in 2021, determine if they are going to issue the 1099-R. If it is a 2021 distribution, and they are not going to issue the 1099-R. In this case you will post the information for the 1099-R to the best of your ability. You will then prepare a substitute 1099-R.
