Did you make any Qualified Charitable Distribution? Do you have any taxable amount in 1099R box 2a for less than the full amount? Is the amount on 1040 line 4a for the gross distribution right? Oh, what codes are in box 7? Maybe one showed up on 1040 line 5 instead of line 4?
