On my state income tax , what is unrecovered contributions for second or future?

Unrecovered contributions are the amounts that you have contributed to all of your Roth IRAs since you started contributing but that you haven't taken out as a distribution.  The taxability of the distribution depends on whether the assets are attributable to pre-tax or after-tax contributions.

 

The uncovered contributions are reported on Form 8606. 

 

Here is a link that explains form 8606.  It is a federal form, but it carries over to the state.
