No withholdings taken from my distribution. So brokerage firm didn't provide a Utah State Tax ID number on the 1099 R and no one can give it to me. Turbo Tax blocking me.

On the Form 1099-R leave boxes 14 thru 16 blank (empty) if there are no state income taxes withheld in box 14.
