My wife received a blank W2 from a previous employer. Everything is blank except for Box 13 - retirement plan. What can I do to e-file? Or, must I file by mail? Thanks

Leave it out. However, you can include this information on your original W-2 and see if it affects any IRA contributions you may have made.
