Any distributions from the IRA after the death of your spouse are payable and taxable income to the designated beneficiary of the IRA. Is someone other than you the designated beneficiary of the IRA? If so, how is it that you were able to obtain a distribution from this IRA after the death of your spouse?

If you name and SSN is on the 1099-R then the account was transferred to you. It is your IRA now.

If your name and SSN is on the Form 1099-R, the payer believed that the payment was indeed payable and taxable to you.

Sounds like OP had the withdrawal check made payable to the Funeral home. However. as you stated, it is still taxable.
