Did you have state income taxes withheld on your 1099-G form?
If not, you do not need to report anything for boxes 10a, 10b, and 11.
If you did have withholding, you will need to contact the Employment Development Department (EDD) to ensure your 1099-G is reported correctly.
Thank you for the reply! I did have state income taxes withheld, but the form does not include Box 11, it only goes up to 4, so there's no way for me to know what that amount is. I have tried contacting EDD about this, to no avail, there is no way to get in contact with anyone. Have you seen a version of this form that only goes up to Box 4?
Here is a page from the CA EDD. It only shows up to box 4 as well.

Make sure there are no additional sides for your 1099-G if you are sure you had state income tax withheld as well.
California does not tax unemployment income so it doesn't make sense for them to withhold state taxes as it isn't taxable within California.
