My 1099r form does not have a distraction code. What do I do?

I took out a hardship on my 401k. The 1099r form provided to me does not have a distribution code in box 7. I have not idea if there are elections or not. How do I know what yo put or if there are exceptions or not?

Could it be code 7 in box 7? Code 7 is the most common code and is the same as the box number. Otherwise you need to get the code and may a corrected 1099R from the plan. Why did you take it out?
