Is Keogh (HR 10) Plan distribution & account balance info reportable on Form 8606 along with IRA info on lines 6 & 7?

Seems like Keogh plan may be considered "deemed IRA"?

Is Keogh (HR 10) Plan distribution & account balance info reportable on Form 8606 along with IRA info on lines 6 & 7?

If the IRA/SEP/SIMPLE box is not marked on the Form 1099-R reporting the distribution from this plan, it is not reportable on Form 8606 and its year-end balance is also not includible on line 6.
