On the form 1099-r, do I need to enter the life insurance payout even if I didn't receive a 1099-r for it?

Is life insurance payout to be entered in 1099-r even if I did not receive a 1099-r listing it?  The company brochure said death claims from the defined contribution plans administered by company are filed directly with the record keepers for the plan and didn't say anything about reporting it in my taxes.

If you did not receive a Form 1099-R for the life insurance payment, do not report the payment as a 1099-R on your tax return.

Life insurance benefits are generally not taxable and no 1099-R is issued.  If you received  payments for a retirement account balance, that should be reported on Form 1099-R and you'll need to include it on your tax return.
