If the only difference is that the State area then enter this as one 1099R with both states listed in the bottom section with their respective amounts. 

After the 1099R entry then carefully answer the questions, as there may be some questions related to each state.  

Now if these two 1099R's have different amounts in box 1, box 2a, then they need to be entered separately.
