If my husband receives SS retirement benefits, but never got a SSA-1099 form, can we base the net on monthly payments x 12?

If so, do we use the monthly amount before deductions or after? (i.e., Medicare)

No you need to get the actual SSA-1099.  Or does he get ssi?  You do not report ssi on your return.  Only SS, SSDI and SSA-1099.

If you cannot find your SSA1099, there is a way to go on the Social Security website and request one. Go to www.ssa.gov You have to create an account with a user name and password.
