If box 12 is blank or zero then ALL boxes 12-14 must be BLANK (not zero).   A zero box 12 means no state tax was withheld so nothing should be reported in boxes 12-14.

Correct. Therein lies the issue. I reported in box 14 the same value as 2a. My box 12 is zero and appropriately left blank. What should I do now? Do I need to amend my 2017 return?

If so, should I just turn in a 1040x to federal, Kansas-where I work, and Missouri-where I live with an explanation statement for what occurred. I also can include the official 2018 copy of my 1099-R from my broker. Anything else?

If box 12 does not have state tax withheld then all boxes 12-14 must be blank - not zero.

A zero in box 12 or a state ID in box 13 will be an error.   *Any* entry in box 12 (even 0) implies that there is state tax withholding.

On the 1099 I submitted box 12 is blank. Which is accurate. I did have MO in 13. I did have the box 2 value in box 14. How big of an issue is this and what should I do to correct it?

If MO is in box 13, remove it.  If no tax in box 12 then ALL boxes 12-14 must be blank.

I have already submitted it with my 2017 return. Do I need to amend that return? If yes, what do I need to send and to whom? A 1040x including an explanation statement to fed, mo-where I live, Kansas-where I work? I can also include the official copy of my 1099 from my broker. Anything else?

No.   It makes no difference on the tax return whatsoever.   It is just to get rid of the internal TurboTax error so you can file.   It is not an actual error at all, just that TurboTax flags it as an error.   If you could file then just ignore it.

Yes this was on my 2017 return and it was accepted federally and by both states. I am not aware of any issues with the returns then or even today. So I’ll just leave it alone and do nothing?
