Only Roth 401(k) or Roth 403(b) contributions are reported on a W-2 and are entered in box 12 with a code of AA for the Roth (401(k) or code BB for the Roth 403(b). Those contributions are not entered anywhere else on a tax return.
Your Roth account in the employer-provided retirement plan is not a Roth IRA.
