I received form 1099-R. Company I worked for is now out of business now. All 401k money from my now former employer I moved for Roth IRA. How to report this form?

You will enter the 1099-R information under Retirement Plans and Social Security. In TurboTax, you will be able to indicate that this was a rollover/conversion so that it is reported and taxed correctly. (See below)
