This is an IRS form 1099-C Cancellation of Debt.
Is this the form that you received?
Why would your previous employer be issuing such a document?
Please clarify.
I got a new job and left the company in June 2022 and they sent that form along with my W2.
im assuming it’s because they are a “Applicable Large Employer”. The back of my form says they are subject to the employer shared responsibility provisions in the Affordable Care Act. I’m just confused because I never signed up for insurance, I only did short time disability because I was pregnant and they did not offer any paid maternity leave or FMLA since I wasn’t there a year before signing up.
As indicated by our wonderful Tax Expert @JamesG1, this is a cancellation of debt if you are giving the correct form number. This form is used for nothing else. If you believe this form was issued to you in error you must contact your prior employer to request a correction.
