You should have received, and need to receive, a 1099-R for this rollover.  It's very important that you enter the 1099-R exactly as it appears on you paper form.  If you did not receive one, call the issuer and request a copy.  The 5498 is for informational purposes only and does not give you all the information you need to enter into the program.

You should have a 1099R from the bank that held the 401k to report that distribution.  You will have taxes to pay on that distribution, since rolling into a ROTH creates tax at the time its rolled into there.
