I got "State Info for Mutual Funds & UTs" in my year end report. Never received before. Do I have to include this info in my Fed Tax? Florida has no state tax

The numbers are a percentage of my 2018 ordinary dividends.

If you've lived in Florida all year, then you don't have to worry about state taxation purposes.
