I didn't receive a form for my Rollover IRA and withdrawal after a job loss. It's also not on the website or the TurboTax tool. What do I do in this situation?

I lost my job and moved my 401(k) into a Rollover IRA, while taking about a 30% withdrawal. The 401(k) was held with the same company as the IRA is. By "TurboTax tool" I mean the too where the app logs in to your IRA account and finds forms for you. It said nothing was available.

Contact the 401(k) trustee for a copy of the 1099-R. They were required by law to issue a 1099-R not later than January 31, 2019.
