The problem is the 1099-R from the Dept of State is different than the 1099-R in TurboTax. Blocks 12-19 are not the same.
Boxes 12 to 19 report your state distributions and taxes withheld on a normal 1099-R form. Often, you won't have state taxes withheld, so if that is the case, you don't need to report anything for that. You should report state distributions in box 16 to make sure your state picks up the distribution, but normally this will occur based on the amount in box 1 and 2(a) on your form 1099-R.
