You enter the Form 1099-R that is reporting the distribution from your Simple IRA and answer the follow-up question that asks what you did with the money. Your choices will include 'converted all or part of it to a Roth IRA'. There will be other questions to answer, but that's all that you need to do.
You do not enter anything in the Roth IRA contribution section because a conversion and a contribution are two different ways to put money into a Roth IRA.
