How do i select the source of this distribution and enter any other New York information for a 1099-R form?

I am submitting a 1099_R form but dont know how to answer the question "Where is your distribution from?" I dont think any of the 4 choices apply so i dont know what to select

If it isn't OPM or Railroad, it is Financial Institution.

Well if it isn't one of the special Retirment systems listed, then it's .... None of the above.
