Once filled out, does the CD version of Turbo Tax actually include the 5329 and explanation in the e-file transmission, or does the form have to be printed and mailed with a paper copy of the return?
You should have used the "Explain" button.
Whenever you use the "Explain" button, the text you enter is included with your e-Filed Return.

Thanks. I did use Explain and typed in my explanation. I have not efiled yet b/c nobody at Turbotax support could confirm that the exile would include it.
If it did not work that way, you would get a warning popping up telling you or forcing you to file on paper, don't you think ?
I have never had to file an explanation.

One would certainly think so, though it would be nice to have someone from Intuit confirm. I got through on the phone and was told that you had to file it by mail. I asked why then would the program say it included it when it pops up and allows you to fill it out, and the rep had no answer.
e-File your return. do not file on paper.
IRS will contact you if your explanation is missing and they want an explanation.
