Yes, Line 5 is the amount that your husband's prior employer paid for health insurance.  There is an exclusion of $3000 if he was a public safety officer.

Box 5 on the 1099R?   You have to enter the 1099R exactly as shown with all the boxes.  Usually box 5 is the difference between box 1 and box 2a and will not be taxable.  

 

On 1040 line 5,  line 5b is the taxable amount.
