Where do I find Form 1099-R? My instructions for mail in say I need to attach one to my 1040, but I can't find it.

Yes, I figured you would as the Teacher Retirement is the 1099R and due to having some income tax withheld in box 4 it needs to be attached.

Do you have any retirement income, a IRA or 401K distribution on the return?

I have Social Security and teacher retirement
