my 1099-R box 16 is empty and turbo tax forces me to review

Potential causes and fixes: 

• Remember, the federal and state already have your tax form, we need to toggle the program
• Box 14 state tax withheld must be less than Box 16 - enter the taxable amount in box 16 (usually box 2a)
• If you have a state id or state income but not both, and no state tax - delete the state section, every line blank - not a zero
• If you live in NY, OH or CA and the above didn't work- completely fill in the state boxes: state, id, income, tax - including using a zero for tax paid
