The plan had until January 31 to mail the Form 1099-R, so either possibly download the form online or wait another few days before contacting the plan to obtain the missing form.
You must enter the 1099R on your tax return. Contact the former employer if you did not receive one yet or if you do not know the contact information for the custodian of the retirement plan. You might need to go on the plan's website to get your 1099R.
I just finally got my last 1099R today 2/4/23. So I would wait another week or so then ask the employer or plan.
