Do I need to input both 1099-R forms?

I received 2 1099-R forms when I entered both, the amount was greater than what I had in the account to begin with. Do I enter both or just one. One has the total amount of my 401 k account. The other has the amount I received from early removal plus taxes and such.

I would contact your Brokerage or the Company listed on the Form 1099-R to determine if they should issue a corrected Form 1099-R.

It sounds like they may have issued one Form 1099-R incorrectly.
