If you have a retirement plan, and took a distribution from it in 2018, 2019 or 2020 then you would answer "yes" to that question.

Yes, this includes all distributions after 2017, which includes early withdrawals. Don't include this year's though if you took one this year. You are reporting this on this year's return.
