I have been using TurboTax for at least 5 years. Always entered data for 1099-R from the form. This year Turbo says I must enter the plan cost at the annuity starting date. It is not on the 1099-R. Where can I find this?

Ok, this is why TurboTax is asking for plan amount contributed, otherwise the entire amount is taxable income, (amount from line 1).
Which could be what you have reported in prior years, and that is why you did not get that question.
Generally, the amount you paid into the plan is listed in box 5.
Go back to the questions again or contact the Plan Administrator to see if they can help with what you put into the plan.
