As part of a divorce settlement, I received a QDRO in the form of a separate 401k. Do I enter it as income? I haven't taken any distributions, and I was told that I only pay taxes on distributions or if I cash it out.
If I do have to enter it, where would I do that?

No. Only when you take a distribution. You only report 1099R you get.
