I deferred a first time 401k RMD for 2018 into 2019. I also took the 2019 401k RMD in 2019 as required. The 1099-R that I received for that account lumped the two RMDs together. Will the IRS figure out that the 1099-R is actually two RMD's or will they be looking for the 2018 RMD separated from the 2019 RMD on the 2019 Return?

The IRS does not need to see the two RMDs separated on the 2019 tax return. Just report the total amount and indicate that you received all of the required distributions.
