In 2020 I made an excess Contribution to my Roth IRA. It was reported as an excess contribution and I received a refund, and a 1099-R in 2021. I reported the 1099-R in 2021, through TT Home and Business. I filled out the form as the 1099-R was reported, and yet while finishing my 2023 taxes the amount of the excess is showing up as a Roth Contribution, and if I try to zero it out through the step-by-step update it doesn't change it to zero. What did I enter wrong in previous years? I don't remember this problem in 2022 but I was surprised to see it this year.

Was I supposed to report the excess refund on line 20 ( 2021 distributions from your Roth IRAs..) of Form 5329-T in 2021? If not where should I have entered it? I noticed the excess has been carried over each year. Should I enter an amended return for 2020, or 2021 - or adjust my 2023 taxes?
To clarify, you made an excess Roth IRA contribution in 2020 and then requested before the due date the withdrawal of the excess contribution for 2020 plus earning?
If yes, then you did no longer have an excess contribution and should not have paid the 6% penalty. If you paid the 6% penalty in 2020, 2021, or 2022, then you will have to amend the returns to remove the penalty.
On the 2020 return, during the IRA contribution interview you will enter that you withdrew the excess contribution on the penalty screen:
In the 2021, 2022 and 2023 returns you will delete excess contribution that got carried over:
The 2021 Form 1099-R for the returned excess contribution plus earnings should have had code P and J in box 7 and therefore, should have been entered on your 2020 tax return.
