I have form 1099-R from my financial institute where I rolled over my Roth 401k to Roth IRA as it is. When I filed my tax it is checking the 4c as rollover with no amount on 4a and adding amount to 5a without checking any option to 5c as result my return was rejected by IRS with reason code : "F1040-478 - If Form 1040, 'IRADistributionRolloverInd', 'IRADistributionQCDInd' or 'IRADistributionOtherInd' is checked, then 'IRADistributionsAmt' or 'TaxableIRAAmt' must have a non-zero value."
I tried to delete and add the from multiple times but still seeing the same on the final review page. How do i procesed from here ?

That is very weird. Haven't seen anyone report that yet. I would try deleting the 1099R and enter it again MANUALLY, especially if you imported it. Oh you said you tried that.

I have the exact the same problem in TurboTax Premier desktop version with the most current updates. Didn’t catch it until IRS rejected my return. Tried deleting, entering again, etc. Every option fails. Trying to reach support now.
I am having the same issue. I had a 401(k) Roth rollover to a Roth IRA (distribution code H). Box 4c was checked as "Rollever" but the gross distribution amount was being put into Box 5a (not 5a). Hence, there was a mismatch between rows 4 and 5. My return was rejected saying:
Im having the exact same problem, Cant get a hold of anyone to help
Just got off the phone with the live tech support. The agent told me they are aware of the problem and working on the fix. ETA to deploy 3/13
I have same issue.
Just talked to Turbo tax help desk. Was told it is a software issue and will be fixed by March 13th. Refile after that date. Thanks.
Thank you. I had the same issue when using the desktop version of TurboTax Premier. No IRA distributions but two 1099-R to report for a 401K to IRA rollover and a Roth 401K to Roth IRA rollover. Line 4 on Form 1040 has the rollover box checked but no amounts. Line 5 on Form 1040 has the rollover box checked and company distributions.
Filed over the weekend and the federal form was rejected for this reason "F1040-478 - If Form 1040, 'IRADistributionRolloverInd', 'IRADistributionQCDInd' or 'IRADistributionOtherInd' is checked, then 'IRADistributionsAmt' or 'TaxableIRAAmt' must have a non-zero value.
Thank you for relaying tech support guidance on the bug and saving us the hold time. I have been ripping my hair out over this today. Hope it works after 3/13.
Same rejection using the online version. "This return cannot be filed at this time due to a processing error that will be corrected on March 13, 2026. On or after this date, open the rejected return, review it for errors, and resubmit the return for electronic filing."

Thanks for letting us know (I had the same issue). Can somebody please confirm, after March 13, that it indeed has been fixed, so that we don't all keep trying to file to the IRS 😉
I also had the same issue. My return was rejected and I received the same message. I'm using Turbotax Deluxe.
The problem appears to be fixed now!
