1099-int form doesn't have any boxes

Hello! I received my 1099-INT form from my credit union but it doesn't look like a normal 1099-INT form. It doesn't have any boxes. It tells me things like YTD taxable dividends and YTD taxable interest as well as finance charges. Is this the information I need to fill in turbotax?

You can use that 1099-INT to report your interest and dividends.   The IRS only needs to know the total interest and the bank name.  For the dividends, the bank should have separated total ordinary dividends (box 1a) and qualified dividends (1b) If not, put the total dividend amount on both places (one is a subset of the other).
