I am a veteran covered by the Department of Veterans Affairs and so is my spouse! Do I need to fill out the 10995-B?

Which state? If your state is a state that requires health insurance and it is asking for information, then yes you will need to fill out the form that it is asking about.

If you are not in a state that requires health insurance and you are both covered by the VA insurance, you do not need to fill anything out.
