rental property and income from insurance claim

I had damage in property which result in renter moving out. Insurance paid me the rental income during that time along with reimbursement for expenses and repairs , couple of questions

 

- do i simply report rent paid by insurance as total rental income ?

- do i have to fill out any other form to report rental reimbursement from insurance? 

- expenses not paid by insurance should be simply reported as expenses?

- do i have to fill out any other form to report rental reimbursement from insurance? no but insurance payments either reduce any repair expenses

- expenses not paid by insurance should be simply reported as expenses? yes
