To report the worthless stock:

  1. Go to Wages and Income
  2. Go to Investments and Savings (1099-B, 1099-INT, 1099-DIV, 1099-K, Crypto)
  3. If you land on the screen Let’s finish pulling in your investment income, select Add Investments.
  4. Select Enter different Way
  5. Select Stock, Bonds, mutual Funds
  6. On the following screen for the Broker name, type in something descriptive, like “Options” in the box, and leave the account number blank.
  7. Answer the questions until you get to the screen Review this xx sale
  8. On the sales section select Long/short term Did not receive 1099-B form
  9. Enter all information and enter 0 for the Proceed.
