Foreign brokerage account - Dividends but no 1099-DIV

I am living in the US and have a brokerage account in Europe. I trade US stocks.

Usually every year they were sending me a 1099-DIV but this year I have not received it and I cannot get a straight answer if and when they will give me one.

I have a report of all the dividends I received and I would like to enter them manually.

However, if I select to do so TT asks me the FEIN and the name of the bank releasing the 1099 (see below). I obviously do not have such information.

How should I proceed?

To avoid the e -file error, I will suggest you to enter under Other Income.  Here are the steps:

In TurboTax online program:

1.  Sign into your account, select Pick up where you left off

2. To the right upper corner, in the search box, type in "other reportable income" and Enter

3.  Select Jump to other reportable income

4.  Next screen, Other Wages Received, select No to continue

5.  Next screen, scroll down all the way to the bottom, under Other reportable income, select Start

6.  Next screen, Any Other Taxable Income? select Yes to continue

This amount will show on line 8 of Form 1040.
