On my 1099b online form theres no shares im trying to fill the discription part should i just put the symbol and company discription.

i thought i was getting a typical 1099b form instead it just has proceeds and cost basis and also was sent a csv docs file

If there is no quantity, don't worry about entering it.  Just enter the name of the asset or ticker symbol, along with the cost basis, proceeds, acquisition date, and sale date.
