Did you sell anything?
If you did not sell anything and have no proceeds then you would not enter the 1099 form on your return.
If you did sell, then you would need to get the information either from your records or by contacting the company to get a corrected 1099.
Your cost basis is what you put into it, so even if that is not on the form, that can come from your records of what you invested. You may be able to find this in an online account summary if you do not have other statements or records.
