In federal return, after you input your 1099B, there will be the next screen and a choice to report state income taxes were withheld on these dividends, check that, hit enter then enter amount and state.
after you input the 1099-Div, check the box "My form has info in more than just these boxes", then the next screen will let you choose "State income taxes were withheld on these dividends (box 12-14 on your 1099-Div), or any other adjustments to the dividends, the next screen lets you report them.
