The statement from TD Ameritrade is a substitute for Form 1099-B. See the FAQ at the link below for instructions to enter it. You have to enter each category separately: short-term covered (box A), long-term covered (box D), and long-term noncovered (box E). If there are a small number of sales, enter each sale separately. If there are a large number of sales, you can enter a summary for each category, but you still have to enter each category separately. Just follow the instructions on the TurboTax screens. There is a drop-down list to select the category. TurboTax will fill out Form 8949 for you and include it in your tax return.

TurboTax does not permit entry of investment sales directly on form 8949. If the sames are summarized by type on the 8949, you can enter the summary of types by choosing that option in the 1099-B section. To find the section, type investment sales in the search box and click on jump to investment sales in the drop down box.

TDAmeritrade might send you a Form 8949, for your convenience, but they are required to send you a 1099-B for your trades through them.   The usual pattern is that you enter your individual trades reported on the 1099-B using the "Stocks, Mutual Funds, Bonds, Other" interview and that information gets summarized on Form 8949.
