Do I have to report separately from the 1099-div the dividends or total capital gain distributions from the mutual fund Capital World Growth and Income Fund-A?
You must report all dividends & cap gains earned in the 2017 tax year from all sources. Not sure what you mean by "separately" ?

So I must enter all 5 as income for tax year 2018. ????

All the income must be reported in the year it is earned.

Do I have to report separately from the 1099-div the dividends or total capital gain distributions from the mutual fund Capital World Growth and Income Fund-A?
If you received one 1099-DIV the funds administrator or your brokerage firm for these distributions, you will enter the information on the 1099-DIV as it is.
Separate distributions listed in the details page do not have to be listed, only the 1099-DIV.
If you received several 1099-DIV, you will enter the information in each one separately.
