If the tax-exempt dividends come from a bond fund, usually the fund administrator includes a list of states in the materials that accompany the 1099-DIV form. If you don't have that list, you can usually find it on the website for the mutual fund company.
Unless you purchased a bond or bond fund for a particular state, it's likely the dividends came from multiple states, in which case you can select More than One State at the bottom of the drop down list.
