I've seen three different recipient name formatting styles and wondering if experts on the forum would share their experience as to which they believe are corrrect/acceptable for an individual beneficiary (non-trust)... Assume the decedent is CAROL DOE and the beneficiary is MARY DOE. Which of the following are acceptable Recipient Name formats on a 1099R for a Traditional Inherited IRA? 1) MARY DOE 2) MARY DOE BENE IRA CAROL DOE DECD 3) CAROL DOE DECD IRA FBO MARY DOE (In all cases the TIN is that of MARY DOE)

The beneficiaries (grandkids) may not now be similarly situated or may become dissimilarly situated in the future, Further, one trust for all four grandkids may not be what @csbarbero's mom envisioned (or were her wishes). Although four trusts would be more of an administrative burden, that setup also provides more flexibility.

It doesn't matter how you get there. The RMD information isn't reported on your tax return unless you indicate that you did not meet your Required Minimum Distribution. So if you met your RMD and the information you input into TurboTax doesn't result in the program thinking you did not meet it, then all is well and good.

The text message you received wasn't from the IRS. The IRS doesn't contact taxpayers by text, or use third party “mediation” agencies, so this was likely a scam.
If you forgot to include 1099-INT income on an already e-filed return, the correct fix is to file an amended return (Form 1040-X).
To file an amendment using TurboTax, you should follow these guidelines.
You must first wait until the initial return is completely processed.
You will have to use the same TurboTax account that you used for the original tax return.
Once you begin your amendment, you'll see your original return.
The refund calculator will start new at $0 and only reflect the changes in the refund or tax due
Only make changes to the areas of your return that need to be corrected.
You have three years from the date you filed your return or two years after you paid the tax due (whichever is later) to file an amendment

2. State retirement plan are usually a defined benefit pension (or annuity). You get a guaranteed income based on years of service, salary and some multiplier. If this is your case, you have an annuity.
You will see the cost on your 1099-R.
Box 1 has the distribution
Box 2 has the taxable amount possibly box 2b is checked with unknown basis
Box 5 shows the amount that is not taxable this year based on your contributions.
Box 9b - if any information at all -has the total you contributed.
Your taxable income should be box 1 minus box 5 = box 2 taxable amount

Thank you for the quick response. I did remove the $777 excess HSA contribution + 19 cents earning from Health Equity in May 2025. I filed Amended tax return (by mail) in June 2025 with a heading "Filed Pursuant to Section 301.9100-2" on the front page of Form 1040-X and paid the additional tax ($186) attributed to the $777 income . I had to manually entered $777 on the Form 8889 smart worksheet to make the numbers work on that form. I MAILED the Amended tax return + the $186 check for the additional tax in June 2025 to the IRS because Turbo Tax couldn't file it electronically probably due to the manually entered Form 8889. The IRS cashed the check within 2 weeks, I received a letter dated 9/15/2025 stating that "We made the changes you requested to your 2024 Form 1040-SR to adjust your other income. As a result you don't owe us any money, nor are you due a refund". I logged onto the IRS website and checked my account and found that my 1040X was accepted and completed. Therefore, I will disregard the 1099-SA that has the $777.19 distribution, code 2. Should I disregard the 1099-R that shows the $8,461.45 Excess ROTH contribution removal, distribution codes J & P because that was accounted for when I e-filed my tax return on 3/24/2025? I haven't done anything yet regarding this 1099-R because I am waiting for 1099s from my other brokerage accounts.

Go back through the PA return where it is asking about your pension. Look for the question that asks what type of plan you have and select "I'm eligible; plans eligible"
This will remove your tax liability for PA if all of your income is from qualified pensions in PA.

If you have Form 8959 on your return and you are not over the threshold for your filing status for additional Medicare tax, please check your W-2 entry and ensure that Box 5 and Box 6 matches your W-2. Box 6 cannot be less than 1.45% of Box 5.
If your W-2 agrees with your input, try deleting the form. Please see How do I view and delete forms in TurboTax Online? for instructions on deleting a form.
